Yorkshire Ambulance Service NHS Trust is investigating two staff members after it says they may have accessed—or attempted to access—medical information about William Chapman, a 24-year-old man who was killed after entering a tiger enclosure at Yorkshire Wildlife Park in Doncaster.

Multiple outlets report that the trust launched the inquiry after receiving emergency calls from the scene on Saturday and after Chapman’s name was later made public. The trust says any access may have occurred “without legitimate reason” or without proper permission, and it has identified two instances involving staff attempting to view the records.

While the reporting is consistent that an internal investigation is underway, outlets differ slightly in wording, describing the concern as possible breaches or attempted access of care records. BBC and other outlets focus on the potential privacy or data breach element, while national newspapers present the same basic facts, including the number of staff involved and that the incident relates to the man’s medical records held by the NHS.
