An accident during a BASE jumping operation in a Utah canyon has killed two people, including extreme athlete Andy Lewis, authorities said. Multiple outlets report Lewis was known for performing onstage with Madonna at the 2012 Super Bowl and for his involvement in high-risk events, including BASE jumping as well as slacklining and tricklining. CBS News and others identify Lewis as one of the two fatalities and say the incident occurred near Moab, Utah, in a remote area of Grand County. Fox News and The Guardian describe the incident as happening during a tandem jump. The sheriff’s office in Grand County is cited as confirming Lewis’s death. One additional victim is reported as an unidentified man. Other coverage also notes Lewis’s prominence in competitive slacklining, including world champion achievements and a Guinness World Record for slackline surfing. The reports do not describe the circumstances of the jump’s failure beyond characterizing it as a “freak” accident and confirming it resulted in two deaths.
